Program Exclusions & Limitations
Youth Plan is intended for ages 13 and younger. The Annual Plan will renew automatically at the end of each contract year. Monthly payment option will incur a one-time $49 enrollment fee and a 5% surcharge. A $250 early cancellation penalty applies if cancelled before the end of the contract year. Cannot be combined with dental insurance or other reduced fee dental plans or financing options.
